Registration with the Practice

New Patients wishing to Register with the practice
New patients wishing to register with the practice will be asked to fill in the required forms above before your registration can be processed.
If you are over the age of 16 you will be asked to attend a Health Check; this will be with the Healthcare Assistant.
You will be registered as a patient of the Practice, rather than with an individual doctor, however you can request a specific doctor if you need an appointment.
Please note if you are on any medication please request extra from your previous GP before leaving that practice, also please provide repeat prescription form so we are aware of the medication you are taking.

When you have completed BOTH forms with the patients details, click on the “Send” button to mail your forms to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.
Alternatively you may print off BOTH FORMS (registration AND HEALTH QUESTIONNAIRE form) fill it out and DROP INTO RECEPTION.
